Health Risk Assessment - Non-Represented Employees
2023 Program Information
The City's Health Risk Assessment (HRA) program for non-represented employees consists of two parts: a Health Assessment survey completed at Go365.com and a Biometric Screening conducted with your Primary Care Provider (PCP).
Employees and spouses who participate in the City's health insurance and who wish to receive a reduced premium contribution for the 2024 plan year (March 1, 2024 through February 28, 2025), must complete both parts of the HRA before December 15, 2023. How to Complete Your Health Risk Assessment
To receive a discount on your 2024 health insurance premium, be sure you and your spouse (if covered on the City's plan) complete the following steps:
- Complete all 6 parts of the Health Assessment survey on Go365.com (Introduce Yourself, Get Active, Eat Better, Reduce Stress, Live Well, and Know Your Health).
- Print the biometric screening form found on Go365.com (select "Activities", followed by "Prevention", then "Biometric Screening"). The biometric screening form is the same for both employees and spouses.
- Schedule a visit with your PCP to complete your Biometric Screening. Remember to bring your biometric screening form with you to your appointment.
- Have your physician's office submit the biometric screening form to Go365.com using one of the methods indicated on the back of the form within 90 days of your visit and prior to December 15, 2023.
What is the Biometric Screening?
The Biometric Screening consists of the following measurements: body mass index (BMI), blood glucose, total cholesterol and blood pressure. In essence, the Biometric Screening provides you with a snapshot of your health. When your Biometric Screening results are submitted to Go365.com, you will receive personalized health tools, from your Go365 Age (an easy-to-understand indicator of your overall health) to recommended activities.
In addition, each year you complete your Biometric Screening, you will earn 2,000 points to use at the Go365 Mall! You will receive additional bonus points for any measurement in a healthy range. Points can be redeemed in the Go365 mall for merchandise, eGift cards, charities, and more. Visit the Go365 webpage for more information.
Note: The HRA program is administered according to federal rules permitting employer-sponsored wellness programs that seek to improve employee health or prevent disease. For more information on your rights in relation to this and other aspects of the City's health plan, review the notifications listed at the bottom of the benefits webpage.
How are Biometric Screening Results Submitted?
Biometric Screening results can be submitted two ways:
- The first way is to ask your physician's office to submit the Biometric Screening form to Go365 using one of the methods on the back of the form. Because the form requires your physician's signature and because lab results may not be completed the day of your visit, this is typically the most convenient method.
- You can also submit the Biometric Screening form yourself by logging in to Go365.com and selecting "Activities", then "Prevention". Next, scroll down to the Biometric Screening tile and select "View Details". Select "Yes" on the Terms and Conditions page. Finally, enter your information into the fields provided, attach the required documentation, and select "Submit".
Note: Your Biometric Screening results must be submitted to Go365 within 90 days of the date of service and must be submitted before December 15, 2023. The date of service must occur on (or after) your effective date with Go365.
Double Check Status
Already completed the Health Assessment and submitted the Biometric Screening Form? Confirm online. Because your results are not shared with Human Resources, you will need to take the following steps if you wish to check your status:
- Log in to Go365.com to access your home dashboard.
- Scroll down to view the “Achievements” tile and select the “View achievements” link.
- You may view the status of your Health Assessment and Biometric Screening on this page (see example image below). Note that due to HIPAA restrictions, your spouse's status will not show a completion date. It may take several days (even a week or longer) for your status to update after a step is completed.
